Facility and practice billing

Billed by the treating provider

The itemized statement from wherever the patient was treated. At a hospital that is room, board, supplies, and the services performed directly. At a clinic, surgery center, or physician practice it is the visit and procedure charges.

ER physician billing

Billed by a staffing group

Emergency departments are commonly staffed by an outside physician group under contract. That group bills separately from the hospital, and the contract can change from one year to the next.

Anesthesiologist billing

Billed by the anesthesia practice

Anesthesia is charged by time and complexity and billed by the practice that provided it. On a surgical case it is frequently one of the larger line items and one of the last to arrive.

Radiologist billing

Billed by the reading group

The facility bills for taking the image. The radiology group bills separately for reading it. Two charges, two entities, one study.

Pathologist billing

Billed by the lab

Specimen analysis is billed by the laboratory or pathology practice that processed it, which is often not the facility where the specimen was taken.

EMS billing records

Billed by the agency or its administrator

Ambulance transport may be billed by a city fire department, a county service, a hospital based service, or a private company, and often through a third party administrator rather than the responding agency itself.

Why is there a separate bill from the ER physician?

Because the physician is often not employed by the facility. Emergency departments are commonly staffed by an outside physician group under contract, and that group bills separately. The same can be true of radiologists, anesthesiologists, and pathologists at a hospital or a surgery center, so one visit can produce bills from several different entities.

Do I need to know which company handles a provider’s billing?

No. Give us the facility and the dates of service and our team handles the discovery of who bills for what. Staffing contracts change, so the group billing for an emergency department this year may not be the one that billed two years ago.

What is the difference between medical records and billing records?

The medical record is the clinical documentation of what was done. The billing record is the itemized statement of what was charged for it. A chart tells you the patient had surgery.
