About DocuSwift Pro

We have worked both sides of the request

A missing record can stall a claim for weeks or push back a filing deadline you cannot move. Our team has spent two decades inside release of information departments as well as record retrieval companies.

That means we know why a request gets rejected before it gets rejected. We know which facilities want a specific authorization format, which ones route through a third party, and who to call when a request has been sitting untouched for nine days.

We work out of Houston, Texas and retrieve records nationwide, from hospitals, clinics, and providers in all 50 states.

The process

Five stages, and you can see yours

These are the exact stages inside your portal. No translation between what we say and what you log in to.

  1. New Intake Review
  2. Follow Up
  3. Records Received
  4. Ready for Delivery
  5. Complete
01

New Intake Review

We validate the authorization before it goes anywhere. Correct signer, correct scope, and confirmation with the facility that the patient was treated there. Most rejected requests fail right here. Catching it now saves you weeks later.

02

Follow Up

The stage where the work actually lives. We stay on the custodian until the records are released, and every contact is logged. You see the current status and the next update date without having to ask for one.

03

Records Received

Records arrive from the custodian and go through a completeness and accuracy check. Nobody tells you they are ready until they have passed it.

04

Ready for Delivery

We confirm the full set is cleared for release and issue your invoice, with an affidavit or certification attached when your matter calls for one.

05

Complete

Your records are downloaded from the portal, encrypted end to end. The request, the file, and its full history stay available to you afterward.

Inside the portal

Records take as long as the custodian takes. Guessing is optional.

Our target is 60 days, and most requests land well inside it. From the day you submit, your portal shows days open, days remaining against that target, and the date of your next update. You never have to email us to ask where something stands.

Common questions

What firms ask us first

The answers people usually have to get on a sales call. Here they are instead.

How long does medical record retrieval take?

Our target is 60 days and most requests land well inside it. Turnaround is set by the custodian, not by us, so from the day you submit, your portal shows days open, days remaining against target, and the date of your next update. You never have to email us to ask where something stands.

Do you require a contract or a monthly minimum?

No. Send us one request or a thousand. Pricing is flat rate either way, with no term to sign and no volume commitment. See pricing.

What happens if you cannot obtain the records?

If a request comes back empty, our retrieval fee is waived. You cover only what the facility charged, at cost, with no markup added.

What states do you cover?

All 50 states. We are based in Houston, Texas and retrieve records from hospitals, clinics, and providers nationwide.

What types of records do you retrieve?

Medical charts, EMS medical records and billing, radiology images, and pathology specimens, plus hospital billing and specialty provider billing from anesthesiologists, ER physicians, radiologists, and pathologists. See all record types.

Are you HIPAA compliant?

Yes. Records are requested, handled, and released under HIPAA, and everything is delivered through our secure client portal rather than by email.
